✔️ Side Effects of Zeropain 30mg/ml Injection

• Common: Dyspepsia, nausea, headache, dizziness.
• Serious: GI bleeding, renal impairment, anaphylaxis, cardiovascular thrombotic events.

✔️ How does Zeropain 30mg/ml Injection work?

Ketorolac inhibits prostaglandin synthesis by decreasing the activity of the cyclooxygenase enzyme.

✔️ Quick Suggestions:

• Should not be used for chronic pain or beyond 7 days.
• Use with caution in patients with renal insufficiency, as it can reduce renal function.
• Increased risk of gastrointestinal ulcers, bleeding, and perforation.
• May increase the risk of thrombotic events (e.g., heart attack, stroke).
• Monitor liver function in patients with hepatic disease.
• Avoid in patients with hypersensitivity to NSAIDs, peptic ulcers, or active bleeding.

✔️ Dosage & Administration: Ketorolac Tromethamine IM/IV Injection

Injection (IV or IM):

Single-Dose Treatment:

• IM Dosing (Adults):
  • <65 years: 60 mg as a single dose.
  • >65 years, renally impaired, or <50 kg body weight: 30 mg as a single dose.
• IV Dosing (Adults):
  • <65 years: 30 mg as a single dose.
  • >65 years, renally impaired, or <50 kg: 15 mg as a single dose.
• Pediatric Dosing (2-16 years):
  • IM: 1 mg/kg (max 30 mg).
  • IV: 0.5 mg/kg (max 15 mg).

Multiple-Dose Treatment:

• <65 years: 30 mg every 6 hours (Max: 120 mg/day).
• >65 years, renally impaired, or <50 kg: 15 mg every 6 hours (Max: 60 mg/day).
• Breakthrough Pain: Do not increase dose or frequency.

Conversion from Injection to Oral:

• Total combined daily dose (oral + parenteral) must not exceed:
  • 120 mg/day for younger adults.
  • 60 mg/day for elderly or renally impaired patients.
• Switch to oral form as soon as feasible.
• Total duration of therapy (oral + parenteral) should not exceed 5 days.

✔️ Interaction

1. Other NSAIDs or Aspirin:
   • Increases the risk of gastrointestinal and other side effects (e.g., bleeding, ulcers).
2. Anticoagulants (e.g., Warfarin):
   • Enhances the risk of bleeding due to synergistic effects.
3. Beta Blockers:
   • May reduce the antihypertensive effect of beta blockers.
4. ACE Inhibitors/ARBs:
   • Increases the risk of renal dysfunction or impairment.
5. Methotrexate:
   • Enhances methotrexate toxicity, including nephrotoxicity and myelosuppression.

✔️ Contraindications:

1. Hypersensitivity:
   • To Ketorolac or other NSAIDs.
2. Age Restriction:
   • Not to be used in children under 16 years of age.
3. Surgical Use:
   • Contraindicated for prophylactic analgesia before surgery due to bleeding risk.
4. Peptic Ulcer or GI Bleeding:
   • Active or suspected conditions are absolute contraindications.

✔️ Pregnancy & Lactation

• Pregnancy:
  • Category C (US FDA): Should be avoided unless the benefits outweigh the risks.
  • Contraindicated during the third trimester due to the risk of premature closure of the ductus arteriosus in the fetus.
• Lactation:
  • Use with caution; insufficient data on excretion in breast milk. Avoid if safer alternatives are available.

• Exercise caution in elderly patients (>65 years) due to increased sensitivity to adverse effects.
• Avoid in patients with active or suspected GI ulcers, bleeding, or hepatic dysfunction.
• Use with caution as NSAIDs may exacerbate asthma symptoms.:
• Discontinue immediately if corneal epithelial breakdown occurs.
• Avoid use while wearing contact lenses.
